TERMS AND CONDITIONS

OFF-CAMPUS DIALING SERVICE:

If I subscribe to the off-campus dialing service, I will be assigned a campus calling card number which will enable me to have off-campus dialing from my residence hall phone as well as public access campus phones located on campus. Sharing campus calling cards is prohibited. I will be responsible for any charges resulting from my campus calling card. If sharing my card number results in abuse or the investigation of abuse, each party involved will be subject to a $25.00 fine plus a time and materials charge for the investigation of the calls. If I should lose my campus calling card, I must immediately notify the STS office. Upon receipt of notification, STS will deactivate my existing campus calling card number and issue me a new one. A $15.00 service fee will be assessed for lost campus calling cards.

I understand that each use of my campus calling card number is an extension of credit. I will be responsible and liable for all calls charged to my account until such time I give notice to STS to deactivate my card number. My telephone statement must be paid in full by the due date on the telephone statement or my long distance service will be disconnected and my account will be subject to a $3.00 late fee. I understand that my telephone statement must be paid in full each month by the due date, whether or not I reach my established credit limit.

$150.00 credit limits are in effect. The University reserves the right to change credit limits as they deem necessary. STS will make a concerted effort to notify me when charges to my account reach 70% of my credit limit and will deactivate my account when I exceed my established credit limit. Credit limit warning notification and account deactivation is a courtesy service and is not guaranteed. I will be responsible for all calls billed to my account.

STS sends out a summary statement each month at the end of the billing cycle. It is my responsibility to notify STS if I have not received my telephone statement. Additional copies of my telephone statement are available for $2.00. I am the only person that can request a copy of my telephone statement. A lead time of 2 business days is required to process such a request.

If my telephone service is terminated for nonpayment, the outstanding balance must be paid in full before my campus calling card number is reactivated. If my account becomes past due, I understand that nonpayment is treated like any other student debt. Sanctions such as withholding grades, registration privileges, diplomas or transcripts will be invoked on all past due accounts. STS reserves the right to refuse service or require a deposit for any individual considered to be a credit risk.

I understand that unauthorized use of a campus calling card to place calls constitutes a theft of services and is a violation of both state and federal law. Fraudulent callers will be subject to criminal prosecution and a STS fraud investigation fee. I understand that STS is not liable for telephone calls fraudulently placed with my authorization card number.

This service may be canceled by either party upon written notice. STS may terminate my service if I fail to abide by the terms of this agreement. If my service is terminated, I understand that I will be responsible for all calls placed with my campus calling card number.

RATES: Rates are subject to change without notice. Current rates are published on our Web site and are available by contacting the STS office.

CAMPUS-ONLY AND OFF-CAMPUS DIALING SERVICE:

I understand that if my check for telephone service is returned by the bank for any banking reason, such as insufficient funds, I will be charged a $15.00 returned check fee. The fee will be applied to my telephone account. I will be notified by Student Telephone Services.

I will notify STS of any change of rooms or any home address or telephone number change.

ACTIVATION FEES: Dial tone activation fees are non-refundable and must be paid at the time of request.

VOICEMAIL: I understand that I should contact the STS office or Telecommunications to have my voicemail forwarded. If I forget my pass code for voicemail, there is a $5.00 reset fee.
